Long Term Condition Management Training

| Through funding from The Big Lottery, our condition-specific training course for Carers is held one morning a week for 6 to 8 weeks. We use professionals from local health, community care and the wider community to present relevant information and create constructive links with Carers. We aim to facilitate good discussion and exchange of experience between Carers throughout the course, and provide opportunities for peer support in the future. We also organise the opportunity, for those of the group who wish to take part, for a digital story to be produced which reflects important aspects of the Carers’ experience. This film can then be used to inform other carers and professionals. |

| We also explore linking with professionals for alternative training opportunities for Carers who have not been able to attend the course. |

Work with GP practices and hospitals
|
| We work with GP Practices to help them build their carer register and increase awareness and support for Carers throughout the Health Services in Moray. We are working to establish a Carer “champion” in each practice and develop improved health screening opportunities and referral systems for Carers. We have also started to introduce the Carers UK GP Toolkit into surgeries. We also raise awareness of Carer issues in hospitals and have produced an information pack to be used on patient discharge. In both areas we are working to ensure information is current and accessible. |

Pharmacy Initiative

| We work with pharmacies throughout Moray as a way of reaching out to hidden and /or isolated Carers. Leaflet and freepost cards are included in medication bags. These inform Carers about our services and give them the opportunity to make contact with our project. Moray Carers Project offers free registration and if required, advice on how to access further support. We are also developing a similar scheme in GP surgeries. |

Information, signposting, events, networking
|

We are always available to Carers for appropriate information, signposting and referrals. We send out newsletters containing local and national information relevant for Carers and hold events and coffee mornings with a focus to inform Carers and bring them together in a social way. We are members of the Carers Service Provider Network and link with other professionals whenever we can. The CSPN members are: |
